Students Off-Campus Activities Safety Management Form

  1. For students who wish to apply for off-campus activities, please fill out this form and submit it to the Campus Safety and Security Center, Military Training Office; for the mountaineering activities, an copy of the activity plan is required additionally.
  2. When the event is over (or the participants have returned home), please report the situation to the on-duty military instructor; in the case of an overnight activity, please do so before 10:00 pm every night (Tel: 04-23323000 ext. 3043).
